Application and Testing Fee: Php 1,200.00 (Non-refundable)

POLICIES ON ADMISSIONS

  1. All applicants to the Grade School are required to undergo the admission screening process.  The screening process will provide the school with data to determine the eligibility of the applicant for acceptance in Xavier School.
  2. Parents will be informed about the result of the screening process.  However, the details of the results will remain confidential.
  3. The applicant is given the admission test once only.  Re-testing is not allowed.
  4. All applicants should have passing grades in all subjects from the previous grade level, and in the First and Second Quarter/Trimester of the current grade level for SY 2022-2023. 
  5. All applicants are required to undergo Vision and Hearing Screening Tests and a comprehensive medical check-up. Results of the tests should be submitted with the other requirements before testing.
  6. The Grade School Unit accepts transferees in Grade 1 to Grade 5 only, depending on the number of available slots.
  7. Application for admissions to Xavier School San Juan and Xavier School Nuvali during the same year is not allowed.

GENERAL NORMS

  1. A candidate has a big chance of meeting the requirements of Xavier School if he has attained a satisfactory evaluation in the following areas:
    1. Attention span
    2. Interpersonal skills
    3. Communication skills in English, Filipino and Chinese
      • Comprehension
      • Expression
    4. Competence in numeracy and literacy
    5. Self-discipline
    6. Stress tolerance
    7. Respect for other people
  2. The school also takes into account the extent to which the applicant’s family environment is open to and supportive of the Catholic Christian values espoused by the school.
  3. The applicant has not been under any major disciplinary status or involved in any major disciplinary case within the last school year.
  4. For applicants to Grade 2 and higher levels, the applicant must have formal instruction in Chinese.

ADDITIONAL REQUIREMENTS FOR FOREIGN APPLICANTS

  1. An applicant coming from a foreign school whose medium of instruction is not English, must undergone at least one (1) school year of schooling in the Philippines prior to application to Xavier School.
  2. The final Report Card/Evaluation Report must be authenticated (Red Ribbon) by the Philippine Embassy of the country of origin as part of the enrollment requirement.

MEDICAL REQUIREMENTS

  1. The applicant must have completed the following immunizations:
    · DTaP : At least 5 doses. The last dose must be given on or after the 4th birthday.
    · Tdap : 1 dose given after the 10th birthday
    · MMR : 2 doses
    · Varicella : 2 doses with last dose on or after the 4th birthday or
    verification of disease.
  2. Results of the Vision and Hearing Screening Test
  1. Accomplished Application Form
  2. Learner’s Reference Number (LRN) from the current school
  3. Original copy of the Applicant’s Birth Certificate
  4. Original copy of the report card of the previous grade level (SY 2021-2022)
  5. Photocopy of the grades in the current grade level (SY 2022 – 2023)
  6. Baptismal Certificate if baptized in the Roman Catholic Church (photocopy verified against the original)
  7. Accomplished Recommendation Form from the current school
  8. Three (3) copies of recent identical 1.5” X 1.5” photos in white background for the following:
    • Application Form (1 copy)
    • Test Permit (1 copy)
    • Health Record (1 copy)
  9. Accomplished Student Health Record Form.
  10. Accomplished Data Privacy Agreement Form.
  11. Result of Vision and Hearing Screening Test
  12. Two (2) copies of recent identical family picture in size 4R (4"x6") - one to be pasted on the application form and the second copy to be submitted upon acceptance to the schoo. 

Note: Only immediate family members should be in the picture. The people in the picture should be wearing decent clothing and should NOT be wearing caps and sunglasses.

Additional Requirements for Naturalized Filipino and Non-Filipino Applicants

Filipino

  • Naturalization Identification Certificate (NID), if naturalized Filipino, or Identification Certificate of Registration (ICR), if born abroad with Filipino Parents (Photocopy)

Non-Filipino (Permanent Resident)

  • Alien Certification of Registration (ACR), (Photocopy)
  • Immigration Certificate of Residence, (Photocopy)

Non-Filipino (Temporary Resident)

  • Alien Certification of Registration (ACR), (Photocopy)
  • Immigration Certificate of Residence, (Photocopy)
  • Special Study Permit (SSP)
  • Passport pages showing the following information:
    • Passport Number
    • Place Issued
    • Date Issued
    • Expiration Date
    • Date of Entry into the Philippines
    • Entry Status
    • Length of Authorized Stay

Exempted from submitting of ACRs are the children of the following:

  • Permanent Foreign residents
  • Aliens with working permit
  • Foreign Diplomat Personnel
  • Personnel from duly accredited international organizations residing in the Philippines
  • Holders of Special Investor's Resident Visa (SIRV)
  • Holders of Special Retiree's Resident Visa (SRRV)

Note: Parents of the abovementioned students should submit to the Registrar's Office a copy of the documents supporting their residence status.

  1. Go to the Xavier School Parents Portal at https://sa.xs.edu.ph/PARENTS_STUDENTS/parents_student_index.htm:
    1. Click on New User? Click here to Register.
    2. Choose Basic Ed (up to grade 10).  Then click Proceed Next.
    3. Click TRANSFEREE then click Register
    4. Choose the appropriate Grade Level. The screen will automatically go to the next section.
    5. Kindly fill out completely all the General Student Personal Information Sheet (GSPIS).  Then click on the button Click to Create Basic Information.
    6. Complete the System Login Information.  Please take note of the Temporary Student Number and Password.  You will need this information to access the Parents Portal for your transactions in Xavier School.
  2. Download the following forms:
    1. Student Health Record Form
      • The Health Record Form should be filled out by your physician (and specialist for children with special medical conditions).  Kindly print the form on a legal size bond paper
    2. Data Privacy Agreement Forms
    3. Recommendation Form
      • Kindly print the form on a legal size bond paper. The Recommendation Form should be filled out by the school head or teacher of your child’s current school. Please request the school head or teacher of your son to email the accomplished form to xssjadmissions@xs.edu.ph.
  3. Upload the scanned copies of the following documents in PDF format on the Admissions Upload Facility:
    1. Application Form with the ID picture of the applicant and Family Picture attached, duly signed by the parents
    2. PSA Birth Certificate 
    3. Report Card/Progress Report for the First Month/Quarter/Semester of the current school year
    4. Baptismal Certificate if baptized in the Roman Catholic Church (original and photocopy)
    5. Student Health Record Form
    6. Vision and Hearing Screening test results
    7. Data Privacy Agreement Forms
  4. Instruction regarding the payment of the application and testing fees will be emailed to the parents of the applicant within five (5) working days upon verification of the application documents. 
  5. The schedule of your son’s test will be sent to the email address you provided the school. 
  1. The test permit will be emailed to the parents of the applicant by the Testing and Research Center upon completion and approval of the submitted documents. This includes the schedule of examination, important guidelines and testing protocols for the onsite testing.
  2. Kindly acknowledge receipt of the test permit by submitting the accomplished Reply Slip/Parental Consent Form through the upload facility provided in the test permit. Note that the submission of the Reply Slip/Parental Consent Form is required.

Please make sure to review the detailed guidelines and reminders prior to the actual test to ensure a smooth and successful testing administration.

1. The admissions test for Grade 2 to 5 will be conducted onsite. Applicants are expected to be at the testing area at least fifteen (15) minutes before the scheduled test. Latecomers will not be entertained.

Note: For applicants to Grade 1, the mode of admissions test is online and will be conducted via Zoom. Kindy refer to your test permit for specific specific instructions and guidelines as well as the link to access the online test.


2. Testing may be rescheduled (and you will be given a new schedule via email) only in the following instances:
   a. The applicant is sick and not well enough to take the test.
   b. There is death in the family.
   c. Xavier School calls off classes/work due to inclement weather or other unforeseen events that could affect the scheduled onsite. We will contact you when this happens.


Note: For rescheduling and other testing concerns, please email the Testing & Research Center at trc-gsadmissions@xs.edu.ph and inform them of your situation.


3. In case testing needs to be rescheduled due to reasons other than those cited above, an additional testing fee will be required and a new test permit will be issued to the applicant.


4. The applicant must strictly follow the test protocols.

  • Results of application will be emailed to the parents of the applicant.  
  • No results will be released by telephone.
  • For applicants who have been accepted, the parents are required to make a reservation to express their intention to enroll their son in Xavier School. 
  • If the reservation fee is not paid within ten (10) days after the release date of the test results, we will assume that you are no longer interested in your child’s admission to Xavier School.  His application will be cancelled automatically.
  • The acceptance letter will include instructions on how to make the reservation and how to pay the required Reservation Fee. For other concerns regarding the Reservation Fee, please feel free to see the School Treasurer.

ENROLLMENT FOR ACCEPTED APPLICANTS

The procedure for enrollment for accepted applicants will be announced before the end of the current school year.

SECTION ASSIGNMENTS FOR ACCEPTED APPLICANTS

  1. The school reserves the right to assign accepted applicants to either a morning or an afternoon session. Priority for the morning session will be given to accepted applicants with older brothers studying in Xavier School.
  2. The section assignments will be posted during the Parents’ Orientation in June 2023.
  3. The mode of education (whether online or onsite) at Xavier School for School Year 2023-2024 will depend on the public health situation of the country. The School will be guided by the advisories of the Department of Education, the Department of Health, local and national government guidelines, and the recommendation of medical experts.
