1. All applicants to the Grade School are required to undergo the admission screening process. The screening process will provide the school with data to determine the eligibility of the applicant for acceptance in Xavier School.

2. Parents will be informed about the result of the screening process. However, the details of the results will remain confidential.

3. The applicant is given the admission test once only. Re-testing is not allowed.

4. All applicants should have passing grades in all subjects from the previous grade level, and in the First and Second Quarter/Trimester of the current grade level.

5. All accepted transferees are placed on probationary status on their first year in the Grade School.

6. The Grade School Unit accepts transferees in Grade 1 to Grade 5 only, depending on the number of available slots.

1. A candidate has a big chance of meeting the requirements of Xavier School if he has attained a satisfactory evaluation in the following areas:

  a. attention span

  b. interpersonal skills

  c. communication skills in English, Filipino and Chinese:

  •    comprehension
  •    expression

  d. competence in numeracy and literacy

  e. self discipline

  f. stress tolerance

  g. respect for other people

2. The school also takes into account the extent to which the applicant’s family environment is open to and supportive of the Catholic Christian values espoused by the school.

3. The applicant has not been under any major disciplinary status or involved in any major disciplinary case within the last school year.

4. For applicants to Grade 2 and higher levels, the applicant must have ongoing formal instruction in Chinese.

1. Accomplished Application Form

2. Learner’s Reference Number (LRN) from the current school

3. Original copy of the Applicant’s Birth Certificate (Philippine Statistics Authority [PSA] Copy for Filipino Citizens).

4. Original copy of the report card of the previous grade level (SY 2018-2019)

5. Photocopy of the grades in the current grade level (SY 2019 – 2020)

6. Baptismal Certificate if baptized in the Roman Catholic Church (photocopy verified against the original)

7. Accomplished Recommendation Form(Download here) from the current school

8. Three (3) copies of recent identical 1.5” X 1.5” photos in white background for the following:

  • Application Form (1 copy)
  • Test Permit (1 copy)
  • Health Record (1 copy)

Note: Scanned and computerized pictures are not accepted.

12. One (1) copy of a recent family picture (4R) to be pasted on the application form.

Note: Only immediate family members should be in the picture. The people in the picture should be wearing decent clothing and should NOT be wearing caps and sunglasses.

Additional Requirements for Naturalized Filipino and Non-Filipino Applicants

ADDITIONAL REQUIREMENTS FOR NATURALIZED FILIPINO AND NON-FILIPINO APPLICANTS:

For Naturalized Filipino,

  1. Naturalization Identification Certificate (NID) or photocopy of Identification Certificate of Registration (ICR) if born abroad with Filipino parents

For Non-Filipino Permanent Resident,

  1. Photocopy of Alien Certificate of Registration (ACR)
  2. Photocopy of Immigration Certificate of Residence

For Non-Filipino Temporary Resident,

  1. Photocopy of Alien Certificate of Registration (ACR)
  2. Photocopy of Immigration Certificate of Residence
  3. Special Study Permit (SSP)
  4. Passport pages showing the following information:
    • Passport Number
    • Place Issued
    • Date Issued
    • Expiration Date
    • Date of Entry into the Philippines
    • Entry Status
    • Length of Authorized Stay

Exempted from submitting of ACRs are the children of the following:

  • Permanent Foreign Residents
  • Aliens with Working Permit
  • Foreign Diplomat Personnel
  • Personnel from duly accredited international organizations residing in the Philippines
  • Holders of Special Investor's Resident Visa (SIRV)
  • Holders of Special Retiree's Resident Visa (SRRV)

Note: Parents of the above mentioned students should submit to the Registrar's Office a copy of the documents supporting their residence status.

REQUIREMENTS FOR FOREIGN APPLICANTS

An applicant coming from a foreign school whose medium of instruction is not English, must undergone at least one (1) school year of schooling in the Philippines prior to application to Xavier School.

The final Report Card/Evaluation Report must be authenticated (Red Ribbon) by the Philippine Embassy of the country of origin as part of the enrollment requirement.

  1. Accomplish the following forms:
    1.  Application for Admission to the Grade School Form (Please check Apply Here Tab)
      1. Print the accomplished application form on a 8.5 X 13 inches sized bond paper.
    2. Student Health Record Form
      1. The Health Record Form should be filled out by your physician.  Kindly print the form on 8.5 X13 inch bond paper. 
    3. Recommendation Form
      1. The Recommendation Form should be filled out by the school head of your child’s current school.  This form must be submitted in a sealed envelope. Kindly print the form on an 8.5 X13 inch bond paper. 
    4. Personal Data Disclosure Terms of Agreement Form
      1. Print the accomplished agreement form on a US Letter (8.5 X 11 inches) sized bond paper.
  2. Attach all requirements on the printed application form (Please refer to Application Requirements Tab.)
  3. Present the accomplished application form and other requirements at the Registrar’s Office.  
  4. You will be issued a payment slip after presenting all requirements and encoding the child’s information.
  5. Pay the application fee of Php 1,300.00 to the Cashier at the ACCOUNTING OFFICE.
  6. Present the Official Receipt at the Registrar’s Office to complete the application process.
  1. The applicant must have completed the following immunizations:
  • DTaP : At least 5 doses.  The last dose must be given on or after the 4th  birthday.
  • Tdap : 1 dose given after the 10th birthday
  • MMR : 2 doses
  • Varicella : 2 doses with last dose on or after the 4th birthday or verification of disease.
  1. Results of the Vision and Hearing Screening Test duly accomplished and signed by an ENT Doctor and Ophthalmologist. 
  1. You will receive through email the Test Permit from the TESTING AND RESEARCH CENTER (TRC). Kindly acknowledge receipt of test schedule and then print the form using 8.5 x 11 bond paper, cut cross-wise and attach the applicant's recent 1.5 x 1.5 photo
  2. Bring the applicant to the TRC on the scheduled testing date.  Make sure to bring the Test Permit.

  • Results of the application will be emailed to the parents of the applicant. 
  • No result will be released by telephone.
  • For applicants who have been accepted, the parents are required to make a reservation to express their intention to enroll their son in Xavier School. 
  • If the reservation fee is not paid within five (5) days after the release date of the test results, we will assume that you are no longer interested in your child’s admission to Xavier School.  His application will be cancelled automatically.
  • The acceptance letter will include instructions on how to make the reservation and how to pay the required Reservation Fee. For other concerns regarding the Reservation Fee, please feel free to see the School Treasurer.

The procedure for enrollment for accepted applicants will be announced before the end of the current school year.
